电视胸腔镜辅助小切口在肺叶切除术中的临床应用

The clinical application of Video-assisted mini-thoracotomy in lobectomy

目的探讨胸腔镜辅助小切口(Video-assisted mini-thoracotomy,VAMT)在周围型肺癌切除术中的应用价值。方法分析我院2008年1月～2010年2月间全麻下行电视胸腔镜辅助小切口肺叶切除术的47例患者(VAMT组)及常规肺叶切除的38例患者(常规组)的临床资料、治疗方法、并发症、围手术期管理等情况。记录并比较两组的手术时间、平均出血量、淋巴结清扫数目、并发症、术后住院时间以及住院费用。结果 VAMT组患者平均手术时间(128±40)min,平均出血量(180±40)ml,淋巴结清扫平均(5.5±4.1)枚/例,术后心律失常发生率12.8%(6/47),术后住院时间(6.2±1.4)d,住院费用(1.4±0.4)万元/例次;常规肺叶切除术组患者平均手术时间(160±30)min;平均出血量(210±40)ml,纵隔淋巴结清扫平均(6.3±3.2)枚/例,术后心律失常发生率28.9%(11/38),术后住院时间(9.1±2.3)d,住院费用(1.2±0.5)万元/例次。结论胸腔镜辅助小切口肺叶切除术是安全有效、费用适中的手术方法,适合基层医院开展。

Objective To investigate the efficacy of Video-assisted mini-thoracotomy in lobectomy.Methods Review the47 cases of Radical resection for lung cancer video-assisted mini-thoracotomy（VAMT group） and 38 cases of routine lobectomy（control group） of lung under general anaesthesia in our hospital from Jan,2008 to Feb,2010.Record the mean operation time,the mean blood loss,the number of resected lymph nodes,the complication,the Length of Stay in postoperative and the therapy costs.Results In VAMT group and Control group,The mean operation time were（128 ± 40） min and（160 ± 30） min,the mean blood loss were（180 ± 40） and（210 ± 40） ml,（5.5 ± 4.1） and（6.3 ± 3.2） lymph nodes were removed,the incidence rate of complication were 12.8% and 28.9%,the Length of Stay in postoperative was（6.2 ± 1.4） d and（9.1 ± 2.3） d,and the therapy costs was（1.4 ± 0.4） and（1.2 ± 0.5） van-dollar.Conclusion VAMT lobectomy of lung;pulmonary lobectomy is a feasible,safe and economic way for lung cancer,and is suitable for primary hospital to carry out.
