5mm巩膜隧道切口无缝线手法碎核白内障摘除后人工晶体植入

摘要 目的:评价5mm巩膜隧道切口无缝线手法碎核白内障摘除后人工晶体植入术的疗效。方法:经颞上或鼻上象限5mm巩膜隧道切口,手法碎核后以晶体圈匙娩出晶体核,再以注吸法吸除皮质并经巩膜隧道植入后房型人工晶体,切口无须缝线自行闭合。共486例602眼。结果:术后平均矫正视力:1周为0.54,2周为0.62,1个月为0.78,3个月为0.81。术后平均散光:1周为1.88±1.12D,3个月0.95±0.50D,与前期常规大切口人工晶体植入比较有显著差异(P<0.01),与同期超声乳化组比较无显著差异,结论:无缝线切口非超声乳化白内障摘除后人工晶体植入术的术后反应轻,并发症少,视力恢复快且稳定,特别是术后引起散光变化较小,在不具备超声乳化设备的基层医院有推广应用价值。 Objective:To evaluate the clinical effects of PC-IOL implantation after cataract extraction with scleral tunnel incision(5mm)sutureless manually fragmentation nucleus techniques.Methods: Go through a scleral tunnel incision(5mm) above the temporal bones or nasal superior quadrant,deliver lens nucleus with lens after manual fragmentation nucleus,suck away cortex and then implant posterior chamber phakic intraocular lens through the scleral tunnel.The cut will heal up by itself without stitching.Totally 486 cases(602 eyes).Results: The average postoperative correction vision was 0.54 to one week,0.62 to two weeks,0.78 to a month and 0.81 to three months,and the average postoperative astigmatism was 1.88 ± 1.12D to one week and 0.95 ± 0.50D to three weeks.Thus it can be seen that the pre-conventional large incision intraocular lens implantation was significantly different.(P0.01,there was no significant difference compared with the same phacoemulsification group.) Conclusions: After the sutureless incision non-phacoemulsification cataract extraction with?intraocular lens implantation,the vision can be rapidly and stably recovered with mild reactions and less complications,particularly the relatively small changes were caused by astigmatism.Thus the method is of widespread usage in basic hospitals which do not have phacoemulsification equipments.
