外伤性胰腺“断裂”临床法医学重新鉴定1例

Traumatic pancreatic "break" to re-identify one case of clinical forensic medicine

目的探讨造成慢性胰腺炎急性发作误诊为外伤性胰腺断裂的原因及如何作出科学的司法鉴定。方法应用循证医学的思维方法全面综合审查和分析病史资料。结果外伤性胰腺断裂诊断与伤后患者临床体征恢复情况不符,CT影像学复查结果提示胰腺主胰管存在走形僵直并扩张,与外伤性胰腺断裂诊断不符。结论外力可能是慢性胰腺炎急性发作的诱因之一,非直接原因,两者之间存在一定因果关系,不宜评定损伤程度。

Objective To investigate the cause of chronic pancreatitis misdiagnosed as acute traumatic rupture of the pancreas causes and how to Forensic Science.Methods Evidence-based medicine way of thinking comprehensive was reviewed and analyzed.Results Traumatic rupture of pancreas in patients with the diagnosis and the clinical signs of recovery after injury,inconsistent,CT imaging review the results suggested that the main pancreatic duct stiffness and expansion of existing out of shape,with the diagnosis of traumatic rupture of the pancreas does not match.Conclusion The external force may be the incentive for acute exacerbation of chronic pancreatitis,one of the reasons for non-direct,causal relationship exists between the two should not be assessed damage.
