Discriminant analysis for prediction of glaucomatous visual field defect

摘要 认识青光眼视野缺损的危险因素对于视功能损害的防治十分重要。研究表明,一旦出现视野缺损,视神经纤维丧失量已达50%,已不是青光眼病的早期。视野检查属心理物理学检测,其可靠性很大程度上依赖于医生的技能和患者的合作。因此,若能正确预测青光眼视野缺损的发生与否,则可及时调整治疗方案,有效地保护患者的视功能。青光眼视野缺损的发生发展受到多种因素的影响,如眼压、视乳头改变、青光眼家族史,以及某些全身情况等等,这些众多因素之间也存在着极其复杂的相互关系。 A prospective study was conducted during a 2.5 year period toidentify factors that influence the development of glaucomatousvisual field defects. In 42 eyes with early primary glaucoma butwithout visual field defect initially, 24 eyes developed glaucomatousvisual field defects in the follow-up period. 16 ocular, systemic variab-les were used in a discriminant analysis to separate 24 eyes with visu-al field defects from 18 eyes without visual field defect. The stepwisediscriminant at the 5% significance level retained eight of 16 vari-ables and succeeded in yielding an overall correct classification of81% of cases. of the 8 variables that were found useful in providingmaximal separation, the most important ones were family historyof glaucoma, the differential light threshold in Bjerrum area, the ve-rtical cup/disc ratio and elevated IOP.
