皮肤软组织扩张术中扩张瓣的血供问题初探

Blood supply to expanded flap in soft tissue expansion operation

皮肤软组织扩张器虽已广泛应用于临床，基础研究也有了长足的进展，但仍有很多问题尚需解决与探讨，既往人们普遍认为组织经过扩张后血供更加丰富，任意型皮瓣可明显增大其长宽比例，但通过我科３８４例实践及结合前人的经验，认为经过扩张后所形成的皮瓣并非单纯的经过延迟的任意皮瓣，扩张后所形成的皮瓣血运状态受诸如扩张后皮瓣的厚度，术后扩张瓣皮肤的紧张度及主要供血血管的通畅情况等诸多因素影响，并提出相应的措施来增强扩张瓣的活力，以减少扩张术的并发症发生率。

Skin soft tissue expander is used in clinics and has many progress in basic study. But many questions should be studied and explored. Blood supply of postexpanding flap was thought to be enriched, its length-width ratio be increased. Aftcr 384 cases of observation, we found that expanded flap was di fferent in many way to ordinary delayed free flap, the flood supply is influenced by many factors,such as thickness and tension of expanded flap, patency of main nutrient vessel and so on. In order to reduce complication, important management to enhance flap activity is mentioned.
