Clinical application of HLA matching in kidney transplantation

摘要 目的:探讨HLA配型在肾移植中的临床应用价值。方法:采用单克隆抗体一步法和顺序特异引物聚合酶链反应技术(PCR-SSP),对78例肾移植受者进行随机性HLA-A、B、DR位点配型。结果:能配上2个以上位点者有59例,占75.6%。供受者之间2~3个位点错配与4~5个位点错配,在移植肾功能延迟恢复及肌酐恢复正常所需时间上,均有显著的统计学差异。结论:HLA配型具有快速、准确的特点,能为肾移植受者选择最佳供体。 Purpose:Discussing the applied value of HLA matching in renal graft.Methods:HLA A,B,DR matching by one step monoclonal antibody and polymerase chain reaction with sequence specific primers (PCR SSP) were made for 78 randomly selected donor recipients of cadaveric renal transplantation.Results:There were 59 with tow or more HLA matched patients ( 75.6 %).The difference in the frenquency of rather slowly recovering of renal function and the time of Cr normaled has been conspicuous between 2~3 HLA mismatch and 4~5 HLA mismatch (P< 0.01 ).Couclusions:The HLA matching had proved a high specificity and rapid method for donor selection and sutied for clinical practice.
