达英-35联合二甲双胍治疗多囊卵巢综合征60例疗效观察

Clinical observation of women with polycystic ovary syndrome treated by combined Diane-35 and metformin

目的:观察达英-35(环丙孕酮/炔雌醇)与二甲双胍联合治疗多囊卵巢综合征(PCOS)的疗效。方法:选择2006年9月—2009年11月在我院诊治的多囊卵巢综合征患者60例,随机分为两组。对照组30例给予达英-35治疗,研究组30例给予达英-35联合二甲双胍治疗,连续治疗3个月。观察治疗前后临床表现、性激素、空腹血糖和空腹胰岛素等变化。结果:两组治疗后痤疮明显改善,月经恢复,血LH、T明显下降(P<0.01),LH/FSH比值降低,空腹血糖无变化。单用达英-35组体重指数和空腹胰岛素无变化,联合用药组体重指数明显降低,空腹胰岛素下降(P<0.05),排卵率、妊娠率都较单用达英-35组高,流产率降低。结论:采用二甲双胍联合达英-35对PCOS进行治疗,能有效降低雄激素水平,改善胰岛素抵抗,疗效优于单用达英-35。

OBJECTIVE To observe the clinical effects of combined Diane-35 and metformin in the treatment of women with polycystic ovary syndrome（PCOS）. METHODS Sixty women with PCOS were randomly divided into two groups as Diane-35 plus metformin （n = 30,12 weeks the study group） and alone Diane-35 （n = 30,12 weeks the control group） during 2006 - 2009. clinical features sexual hormone level fasting blood glucose and fasting insulin were evaluated at baseline and after 12 weeks of treatment. RESULTS The seium luteinizing hormone （LH）, testosterone level （T） decreased significantly （P〈0. 01） , and LH/FSH ratio decreased significantly in both groups, and menstrual cycle and acne both improved , there was no significant chang in the fasting blood glucose after treatment, there was no significant chang in body mass index （BMI） and fasting insulin in the control group. The BMI and lasting insulin level decreased significantly in the study group（P〈0. 05）. Ovulation rate, the pregnancy rate markedly increased, and the abortion rate decreased. CONCLUSION The combination of Diane-35 and Metformin for PCOS can not only lead to decreased T level and also improve the insulin resistance. The clinical effects of combined Diane-35 and Metformin is better than alone Diane-35.
