胃巨大溃疡手术治疗与保守治疗临床疗效的研究

Surgical Treatment of Giant Gastric Ulcer and the Clinical Efficacy of Conservative Treatment

目的:比较内、外科手术治疗胃巨大溃疡的的临床疗效。方法:治疗128例经胃镜证实的胃巨大溃疡病例,随机分为两组,手术组64例,保守组64例。保守组患者给予常规内科止血、胃黏膜保护、制酸等治疗。手术组患者采用外科手术治疗。结果:手术组胃溃疡治愈率为88.3%,优于内科保守组的78%(P<0.05),手术组6个月和12个月溃疡复发率、并发症的发生率、癌变率均明显低于内科保守组(P<0.05);出血、穿孔发生率明显降低(P<0.05)。结论:对于胃巨大溃疡外科手术治疗优于传统的内科保守治疗。

Objective：To compare,the surgical treatment of large deers of the stomach caneer.Methods：128 eases treated by endoscopy confirmed eases of giant gastric ulcer were randomly divided into two groups,64 cases of surgical group,conservative group of 64 eases. Conservative group were treated with conventional medicine to stop bleeding,gastric mueosal protection,acid and other treatment. Surgery group were treated with surgery.Results：The surgical cure rate was 88.3% gastric ulcer group,better than the medical conservative group 78% （P〈0.05）,operation group for 6 months and 12 months, ulcer recurrence rate, complication rate, significantly lower cancer rates conservative group in internal medicine （P〈0.05）;bleeding and perforation were significantly lower （P〈0.05）.Conelusion：Surgicat treatment of giant gastric ulcers than traditional medical therapy.
