232例临时起搏器在体外循环术后的应用与管理

摘要 目的:总结我科232例体外循环术后临时起搏器的应用及管理。方法:应用MEDTRONIC5348型起搏器,心脏复跳后常规放置临时起搏导线于右室心外膜表面,按需调整起搏频率、起搏器输出电流及心室感知灵敏度。术后注意观察起搏效果及防治并发症,做好起搏器携带期间的护理。结果:起搏器工作正常,能够维持适宜的心率和有效起搏。全组232例患者,222例术后成功脱离起搏器康复出院。结论:体外循环术后常规放置心外膜起搏导线,必要时进行临时起搏,做好起搏器携带期间的调试及护理,可明显缓解各种缓慢型心律失常带给患者的不利影响,有助于患者安全渡过围手术期。 Objective: To sum up the application and nursing experience of 232 cases temporary pacemaker after cardiopulmonary bypass. Methods: Temporary pacing wire was placed in the right ventricular epicardial surface after heart resuscitation and adjusted pacing frequency, output current and sensing sensitivity on demand by using MEDTRONIC5348 pacemaker. During the period of carrying pace- maker good care was performed to observe the effect of postoperative pacing and prevention of complications. Results: The pacemaker was in working properly and could maintain appropriate heart rate and effective pacing. 222 patients were successfully detached from the pacemaker. Conclusion: Proper using epicardial pacing lead and good nursing after e, ardiopulmonary bypass can significantly alleviate adverse effects of various arrhythmias in patients and assist patients tide over periopcration safely.?
