补肾固涩法合温箱灸治疗中风后急迫性尿失禁的临床研究

Clinical Research of Kidney-tonifying and Astringency-inducing Therapy Combined with Warm-box Moxibustion in Treating Post-stroke Urge Urinary Incontinence

【目的】观察中药补肾固涩法合温箱灸配合膀胱训练治疗中风后急迫性尿失禁的临床疗效及安全性。【方法】将78例患者随机分为试验组和对照组2组。试验组40例采用中药补肾固涩法合温箱灸配合膀胱训练治疗,对照组38例采用中药安慰剂配合膀胱训练。2组均以14 d为1个疗程,连续治疗2～3个疗程。观察2组临床疗效和尿失禁程度评分情况及其安全性。【结果】试验组与对照组的总有效率分别为87.5%、68.4%,2组比较差异有显著性意义(P<0.01)。治疗后2组尿失禁程度评分均有显著改善(与治疗前比较,P<0.01),其中试验组的改善作用优于对照组(P<0.01)。试验期间未发生局部烫伤、疼痛加重及盆腔功能紊乱等现象。【结论】补肾固涩法合温箱灸治疗中风后急迫性尿失禁有良好的疗效,且安全性良好。

Objective To investigate the clinical effect of kidney-tonifying and astringency-inducing therapy combined with warm-box moxibustion in treating post-stroke urge urinary incontinence. Methods Seventy-eight post-stroke urge urinary incontinence patients were randomized into two groups. The treatment group （N = 40） received kidney-tonifying and astringency-inducing therapy combined with warm-box moxibustion and bladder training, and the control group （N = 38） received herbal medicinal placebo and bladder training. Fourteen days constituted one treatment course, and the two groups were given 2 - 3 courses of treatment. The therapeutic effect was evaluated, and the scores of urinary incontinence degree as well as the safety were examined in two groups after treatment. Results The total effective rate was 87.5% in the treatment group and 68.4% in the control group, and there was statistically significant difference between the two groups （P 〈 0. 01 ）. The scores of urinary incontinence degree were improved in both groups （ P 〈 0. 01 compared with those before treatment）, and the improvement was obvious in the treatment group （P 〈0. 01 compared with that in the control group）. During the treatment, there was no local scald, aggravation of pain, or disorder of pelvic cavity function in two groups. Conclusion Kidney-tonifying and astringency-inducing therapy combined with warm-box moxibustion is effective and safe in treating post-stroke urge urinary incontinence.
