保守及手术治疗股骨粗隆间骨折88例疗效分析

Clinical effect analysis of 88 cases of intertrochanteric fractures by open reduce internal fixation and internal medicine

目的 分析骨质疏松性股骨粗隆间骨折的治疗情况，探讨骨质疏松性股骨粗隆间骨折的治疗策略。方法选择2004年1月至2008年6月治疗骨质疏松性股骨粗隆间骨折88例进行回顾性分析，保守治疗12例，手术治疗76例。所有患者同步用药物治疗骨质疏松症。结果随访时间12～42个月，平均31个月。无感染及骨折不愈合发生。4例出现髋内翻。髋关节功能评定（HarrisHip Score）优良率为86．4％。结论要根据不同的适应证及术者的经验，准确预测髋部骨折的手术风险，从而合理地制定治疗策略。

Objective To analyze and investigate the clinical effects of the treatment of osteoporotic intertrochanteric fractures. Methods There were 88 cases of intertrochanteric fractures treated in our hospital from Jan. 2004 to Dec. 2008.12 cases were treated with conservative therapy. 76 eases were operated. Results The patients were followed up for 12 -42 months, with an average of 31 months. There were not any infection and nonunion cases. Coxa vara was found in 4 cases. The excellent and good rate of hip joint function was 86.4%. Conclusion It can be used to predict the risk for the operation of the elderly hip fracture patients accurately. Therefore, the rational decision can be made for the operation.
