手术内固定治疗胫骨平台骨折

摘要 目的探讨胫骨平台骨折手术治疗的方法和效果。方法总结分析胫骨平台骨折手术治疗150例,术中合理选择手术入路,良好复位,修复胫骨平台关节面,进行可靠的固定,植骨,修复损伤的半月板和韧带,术后早期的功能锻炼。结果术后随访8个月~6年,平均18个月,达到骨性愈合时间4~13个月,平均为8个月。参照Lysholm评分标准[1],根据骨折复位情况、愈合情况、功能恢复情况及并发症等条件评分,优114例,良27例,可4例,差5例,优良率为94%。结论治疗胫骨平台骨折,选择合适的手术时机及手术适应证,尽量恢复骨与关节的基本外形结构、轴线等,保证关节的稳定性,术后早期进行功能锻炼,可以明显的改善胫骨平台骨折的治疗效果,恢复关节的功能,减少并发症的发生。 Objective To investigate tibial plateau fracture treatment methods and resuhs. Methods Analyzed 150 cases of Tibial plateau fracture surgical treatment, reasonable surgical approach during surgery, good reduction, repair of tibial plateau articular surface, fixed reliably, bone graft, repair meniscus and ligament injury, early postoperative function of forging. Results Followed up for 8 months to 6 years, an average of 18 months, the osseous healing time was 4 to 13 months, an average of 8 months. Refer to Lysholm^[1] score standard, according to fracture reduction, wound healing situation, function and complications rates and other conditions were excellent in 114 cases, good in 27 cases,4 cases and poor in 5 cases, good rate of 94%. Conclusion Treatment of Tibial plateau fracture, select the appropriate timing of surgery and surgical indications, as far as possible to restore the basic shape of bone and joint structure, axis, etc. , to ensure joint stability, early postoperative functional exercise,can significantly improve the treatment of tibial plateau fracture effect,restore joint function and reduce comnlications.
