早产儿低出生体重儿甲状腺功能检测及临床意义 被引量:3

摘要 目的探讨早产儿低出生体重儿早期甲状腺功能的变化特点及其临床意义。方法以2008年7月~2009年6月我院新生儿科收治的早产低出生体重儿60例作为实验组,同期出生的7日龄正常足月儿60例为对照组,采用化学发光法分别检测实验组生后第1、2、4周血清T3、T4、TSH、FT3、FT4水平,对照组生后第1周血清T3、T4、TSH、FT3、FT4水平。结果早产儿生后第1周血清FT3、FT4水平明显低于足月儿组(P〈0.01);随着日龄增长,早产儿T3、T4、FT3水平显著上升(P〈0.01)。实验组生后1周低T3综合症发生率83.33%,暂时性低甲状腺素血症发生率17%,随周龄增加其发生率逐步下降,至第3周分别为33.33%为5.00%。结论早产儿下丘脑-垂体-甲状腺轴发育不成熟,生后甲状腺功能可降低,危重儿下降更明显,生后1周检测甲状腺功能是必要的,早产儿甲状腺功能低下是暂时的。 Objective To explore the change and the clinical significance of observation on thyroid function on premature delivered low birth weight infants. Methods Serum triodothyrorine (T3),thyroxine (T4),thyroid-stimulating-hormone (TSH),free-triodothyrorine (FT3),free-thyroxine (FT4)were detected by electrochemistry luminescence(ECL) in 60 normal neonates on the 1st week(control group) and 60 premature delivered infants on the 1st, 2nd and 4th weeks (study group) from Jul 2008 to Jun 2009. Results The concentrations of serum FT3 ,FT4 on the 1st week of premature infants were lower than those on normal neonates (P〈0.01).The concentrations of serum T3 ,T4,FF3 were higher than before significantly as time wen by (P〈 0.01). Conclusion The thyroid function on the premature delivered infants is lower.h is more seriou in critical premature delivered infants.h is necessary to detect the thyroid function on the 1st week.The hypothyroxinemia of premature delivered infants is temporary.
