胃黏液腺癌的影像与临床分析

Imaging and clinical analysis of mucinous gastric carcinoma

目的:提高对胃黏液腺癌的认识,探讨其影像学表现与病理及生物学行为的相关性。方法:搜集一年半来经手术及病理证实的胃黏液腺癌12例,术前均应用西门子双源螺旋CT行三期扫描,分析其影像学特点,并探讨影像学表现与病理及生物学行为的相关性。结果:本组9例发现时已为晚期,且肿瘤巨大,部分明显侵犯周围脏器并伴有局部淋巴结转移,其中8例只能行姑息性切除。结论:胃黏液腺癌发病隐袭,多数发现时已经有周围器官侵犯及淋巴结转移,影像学表现特殊,病变多数强化不明显,部分瘤体有点状、结节状钙化,预后较差。

Objective： To improve the recognization on gastric mucinous adenocarcinoma, and study the correlation between the imaging manifestations and pathological characteristics, biological behavior. Methods： Collected 12 cases of gastric mueinous adenocarcinoma over the past year confirmed by surgery and pathology. All the cases underwent tri-phase scanning with Sie- mens dual-source spiral CT preoperatively, the correlation between the pathological and biological behavior was analyzed. Resuits： Nine carcinoma were advanced when found, the sizes of the tumors were huge and the surrounding organs were involved, 8 patients with palliative resection. Conclusion： The incidence of gastric mucinous adenocarcinoma is implicit. Most have al- ready had invasion of the surrounding organs and lymph node metastasis when found. The imaging performance is special, the majority of lesions are not obviously enhanced, part of the tumor with punctiform and nodular calcification. The prognosis of gastric mucinous adenoeareinoma is poor.
