苦参素胶囊治疗慢性乙型肝炎的疗效观察

目的探讨苦参素胶囊治疗慢性乙型肝炎的疗效及免疫机制。方法选择120例慢性乙型肝炎患者:血清ALT异常,但小于正常值2倍;HBeAg(+);血清HBVDNA>105copies/mL。按照随机数字表分为治疗组与对照组。对照组59例单纯予以常规保肝药物治疗,治疗组61例在对照组给药的基础上加用苦参素胶囊200mg/d,3次/d。疗程均为12周。观察两组在用药后ALT情况、乙肝病毒e抗原(HBeAg)、HBeAg/抗HBe血清转换、乙肝病毒脱氧核糖核酸(HBV-DNA)转阴率及外周血T细胞亚群情况。结果治疗组在改善肝功能、乙肝病毒e抗原(HBeAg)转阴率、HBeAg/抗HBe血清转换率、乙肝病毒脱氧核糖核酸(HBV-DNA)转阴率方面明显优于对照组。两组治疗均对T细胞亚群有调节作用,能使CD4+上升,CD8+下降,CD4+/CD8+上升,但治疗组优于对照组(P<0.05)。结论苦参素胶囊可以提高慢性乙型肝炎患者细胞免疫,具有抗HBV作用。可应用于临床上尚未达到核苷类抗乙肝病毒药物使用指针的慢性乙型肝炎患者(肝功能ALT异常,但小于正常值2倍;HBeAg(+);血清HBVDNA>105copies/mL)的抗病毒治疗。
