阴道彩超检查对诊断输卵管间质部妊娠的临床应用

Clinical value of transvaginal color Doppler ultrasound in diagnosis of interstitial tubal pregnancy

目的探讨彩色超声对输卵管间质部妊娠的诊断价值。方法回顾性分析36例输卵管间质部妊娠的超声声像图特征及手术效果。结果超声声像图表现为典型孕囊型(5例)、不典型孕囊型(6例)和包块型(25例)三种类型;36例输卵管间质部妊娠患者中超声正确诊断30例,误诊为宫角部妊娠3例,双子宫一侧妊娠1例,滋养细胞疾病1例,残角子宫1例。结论经阴道彩色多普勒血流显像对输卵管间质部妊娠的诊断是一种可靠的诊断方法,具有重要的临床价值。

Objective To explore the clinical significance of transvaginal color Doppler ultrasound（TVCDU） in the diagnosis of interstitial tubal pregnancy.Methods 36 cases with interstitial tubal pregnancy confirmed by surgery and pathology were retrospectively analyzed according to the characteristics of two-dimensional ultrasonography together with TVCDU and Postoperative pathological diagnosis.Results 36 cases of interstitial tubal pregnancy were classified into three group： typical ultrasonographic features of normal pregnancy but mal-positioned（5 cases）;atypical ultrasonographic features of normal pregnancy and mal-positioned（6 cases）;abnormal masses adhere to the horn of the uterus（25 cases）.30 patients were accurately diagnosed in 36 cases of interstitial tubal pregnancy.3 cases of interstitial tubal pregnancy were misdiagnosed as cornual pregnancy,1 cases as didelphic uterus with one uterine pregnancy,1 case as gestational trophoblastic disease,the rest 1 cases as rudimentary horn of the uterus.Conclusions TVCDU is a feasible way with important clinical value for diagnosis of interstitial tubal pregnancy.
