鼻胃镜检查在老年人上消化道疾病中的应用

Preliminary Appreciation of the Value of Transnasal Gastroscopy in the Diagnosis and Treatment of the Upper Gastrointestinal Diseases

目的探讨鼻胃镜在老年人上消化道疾病中的应用价值。方法对100例老年人采用EG530N型鼻胃镜进行观察。在检查前、中、后进行心率、血压、脉搏氧饱和度监测。结果 30例经口插入,70例经鼻插入,全部顺利插入完成检查过程,并能清晰辨别黏膜病变。91例(91%)基本无反应,7例(7%)有轻度恶心反应,1例(1%)反应较大,1例(1%)鼻黏膜出血。在检查前、中、后心率、血压和SpO2几乎无变化。结论鼻胃镜由于外径细小,检查痛苦小,患者易于接受,更适合于老年人,对上消化道疾病的诊断有很大的应用价值。

Objective To investigate the safety and effectiveness of nasal endoscopy in elderly patients with upper digestive tract diseases. Methods A group of 100 patients over 65-year old were involved in this study from May 2008 to October 2009 in our hospital. The biochemical data were checked, and the changes of vital signs were examed in all patients. Results 70 patients received the examination through nose and 30 via mouth. Nine cases had endoscopy-related complications without death. There were significant differences in index when compared pre-and post-operationally. Conclusion Most patients tolerate ultrathin EGD with significantly shorter recovery time and less overall anxiety than with the conventional procedure. Techniques to reduce gagging and choking associated with ultrathin EGD may improve patient acceptance and tolerability. Adoption of ultrathin EGD by primary care physicians may decrease cost, time, and inconvenience while increasing access to EGD for many patients.
