复发性星形细胞瘤的再手术治疗

Reoperation in the treatment of recurrent astrocytoma

目的：探讨复发性星形细胞瘤患者再手术效果。方法：本组星形细胞瘤（Ⅲ～Ⅳ级）再手术治疗３１例，与第一次手术间隔期为２～１５０周。再手术前Ｋａｒｍｏｆｓｋｙ分级（ＫＲ）为３０～９０，７例接受３次手术。结果：再手术后存活期为１～１８２周，２５例ＫＲ术后１个月较术前明显改善，术后肿瘤全切除者术后生存期较相应组明显延长。结论：再手术治疗复发性星形细胞瘤是可行的，术后ＫＲ≥６０，手术间隔＞６个月及术中肿瘤全切除者。

Objective:To study the effect of reoperation in the treatment of recurrent astrocytoma.Methods:31 patients with recurrent astrocytoma were reoperated.The interval between the first operation and reoperation was 2～150 weeks.The Karmofsky rating(KR)before reoperation ranged from 30 to 90.Seven patients had more than one operation.Results:The survival time after reoperation was 1～182 weeks.Twenty five patients improved in KR 1 month after reoperation.The patients with total removal of the tumor lived longer than their respective counterparts.Conclusions:Reoperation is feasible for the treatment of recurrent astrocytoma in patients with KR≥60,and with interval between operations>6 months.Patients with tumor total removal have better results.
