彩色多普勒超声动态观察对围生期脑损伤的应用研究

摘要 目的:探讨彩色多普勒超声动态观察对不同时期围生儿脑损伤的应用价值.方法:应用彩色多普勒超声检查动态观察窒息足月儿缺氧缺血性脑病(HIE)78例,早产儿脑白质软化(PVL)65例,极低体重儿(VLBWI)脑室内出血(HIV)35例,正常对照组100例的脑血流动力参数变化及二维超声改变. 结果:头颅彩色多普勒超声检测HIE组、PVL组及VLBWI组,ACA、MCA、PCA的Vs、Vd及Vm均降低,与对照组比较,有统计学意义(P〈0.05).HIE组PI、RI明显增高,与对照组比较,有统计学意义(P〈0.05).足月儿缺氧缺血性脑损伤二维超声表现为脑实质回声弥漫性增强,脑结构模糊伴侧脑室裂隙变窄或消失.早产儿白质损伤及脑室内出血程度与胎龄、体重有关.结论:彩色多普勒超声动态监测不同时期围生儿脑损伤的脑血流动力学改变,可获取早期脑损伤时的重要参数,对预测脑损伤程度,评估预后有较好临床意义. Objective:Discussion colored Doppler supersonic dynamic observation encircles to the different time lives the application value which the brain damages. Methods: Applies the colored Doppler supersonic inspection dynamic observation to suffocate the full--term oxygen deficit anemic brain illness (HIE)78 example,the premature infant brain white matter softens the (PVL)65 example, extremely low body weight son (VLBWI) ventricles of the brain internal hemorrhage (HIV) 35 examples,normal control group 100 example brain blood stream dynamic parameter changes and two--dimenslonal supersonic change. Results: Head colored Doppler supersonic examines the HIE group, the PVL group and the VLBWI group, ACA,MCA,PCA Vs,Vd and Vm reduces, compares with the control group, has statistics significance (P〈0. 05). HIE group PI, the RI obvious markup, compares with the control group, has statistics significance (P〈0.05). The full--term oxygen deficit anemic brain damage two--dimensional supersonic performance strengthens for the brain essence echo diffused,the brain structure fuzzy partner ventriculi laterals eneiphali crevasse stricts or vanishing. The premature infant white matter damage and the ventricles of the brain internal hemorrhage degree and the embryo age, the body weight concern. Conclusion:The colored Doppler supersonic dynamic monitor different time encircles lives the brain blood stream dynamics change which the brain damages, may gain the early brain damages when the important parameter, to forecast the brain extent of damage, appraised the prognosis has the good clinical significance.
