摘要
对10例心内膜弹力纤维增生症(EFE)患儿的多普勒组织显像(DTI)结果进行了分析。发现本组二尖瓣环收缩期及舒张早期运动速度(MVR-SV及MVR-DeV)、左室后壁心内膜收缩期及舒张期运动速度(LVPW-EnSV及LVPW-EnDV)均显著降低,其阳性率高于传统的心功能指标;另外,患儿的二尖瓣血流舒张早期峰值流速(E峰)与舒张晚期峰值流速(A峰)之比显著降低,这对鉴别EFE与扩张型心肌病(DCM)有重要价值(后者E/A正常或升高)。
To evaluate the use of left ventricular motion velocity in diagnozing and determining the affected locus of endocardial fibroelastosis(EFE),and differenting EFE from dilated cardiomyopathy(DCM) 10 patients with EFE and 143 normal children were examined by Doppler tissue imaging(DTI) and transmitral early(E) and late(A) Doppler flow velocity The DTI indicators including mitral valvula ring systolic velocity(MVR SV) and diastolic early velocity(MVR DeV),the endocardial systolic and diastolic velocities of left ventricular posterior wall(LVPW EnSV and LVPW EnDV ) The MVR SV,MVR DeV,LVPW EnSV and LVPW EnDV were all signific antly decreased in patients with EFE than those in normal subjects(respectively3 00±0 90 versus 7 41±0 68,4 33±1 03 versus 12 97±0 90,4 50±1 21versus 7 15±0 68,5 00±1 32 versus 15 37±1 17) The abnormal rate of DTI was significantly higher than that of conventional left ventricular function indicators DTI provide a clinically useful method for diagnosis and determinating the affected locus of EFE
