摘要
目的:探词血浆甘油三酯水平与胰岛素敏感性的相关关系。方法:对新疆地区万人糖尿病普查中做口服葡萄糖耐量实验( O G T T)1028 例的临床资料进行分析。结果:单相关分析显示血浆甘油三酯水平与胰岛素敏感指数( I A I)呈显著负相关(r= 0.35, P< 0.0001)。以方差分析判定 I A I逐步增高的4 组中血浆甘油三酯水平呈逐步降低( P< 0.05)。逐步回归分析显示甘油三酯水平与 I A I呈负相关,与体重指数( B M I)及腰、臀围比值( W H R)呈正相关。调整年龄、性别与肥胖诸因素影响后, I A I逐步升高的4 组间血浆甘油三酯水平(分别为 215,155,137 及1.27 m m ol/ L)呈逐渐减低。结论:本组人群中血浆甘油三酯水平变化与胰岛敏感性密切相关,并随着胰岛素抵抗的增加而升高。高甘油三酯血症确为胰岛素抵抗的表现,应包括在胰岛素抵抗综合征中。
Objective: To investigate the relationship betwecn thc level of plasma triglyceride and insulin sensitivity. Methods: OGTT was performed in l028 persons and plasma triglyceride level was tested during diabetic mellitus investigation in Klamayi, Xinjiang. Results: With spearman correlation ana1ysis anegative relation was found between plasma triglyceride and insulin sensitivity (r=-0.35,P<0.001);using analysis of variance, it showed that there was significant difference in plasma trig1yceride lcvel with different IAI,P<0.05; In a forward stepwise multiple logistic regression ana1ysis, IAI was negative1y correlated with plasma triglyceride. Body Mass lndex (BMI), ratio of circumfercnce of the waist to hip (WHR) was positively correlated with plasma triglyceride. After adjustment for age, sex and obesity, plasma triglyceride level was decreasing with thc increase of IAI. Conclusion: In this population, the change of plasma triglyceride level was closely related with insulin sensitivity, it was arose with insulin resistant increased. Therefore, hypertriglyceridemia is a part of insulin resistant syndrome. it should be included in insulin resistant svndrome.
