STUDY AND CLINICAL APPLICATION OF INTERSTITIAL BRACHYTHERAPY WITH IRIDIUM 192 FOR GLIOMA

摘要 为观察用192Ir后装治疗脑胶质瘤间质内放疗的有效性及其并发症,用皮下荷瘤鼠和脑荷瘤鼠模型行间质内放疗实验,证明在内照射后可抑制肿瘤生长及延长鼠的生存期的可靠基础上,对72例脑胶质瘤采用192Ir后装机行瘤间质内近距离照射,取得良好疗效。手术无何严重并发症,无死亡。术后随访1~5年,生存率分别为92%、82%、72%、56%及32%,表明生存期显著延长。结论:脑深部胶质瘤用192Ir核素瘤间质内高剂量率照射可有效抑制肿瘤生长,控制复发,延长生存期是一种可供选择的有效的方法。 To determine the effectiveness and toxicity of interstitial brachytherapy with 192 Ir on the brain malignant gliomas, 72 patients with anaplastic astrocytoma and multiform glioblastoma were treated with stereotactic brachytherapy by using temporary high activity 192 Ir sources. The median survival for patients underwent brachytherapy was 36 months. Survival rates at 1, 2, and 3 years after brachytherapy significantly increased (92%, 82% and 72% respectively). It was suggested that adjunctive treatment of brain malignant tumors with discrete high doses of radiation therapy delivered by 192 Ir brachytherapy might improve local tumor control and prolong survival time.
