儿童肱骨髁上骨折肘内翻畸形的预防及治疗

Experience of prevention and treatment to child humeral supracondylar fracture and cubitus varus

目的:评价不同方法治疗儿童肱骨髁上骨折的疗效,探讨肘内翻的预防及手术治疗疗效。方法:对122例儿童肱骨髁上骨折患者及22例行肘内翻畸形手术的患者随访进行临床评估。结果:122例肱骨髁上骨折患者中,手法复位组优良率50%,闭合穿针内固定组78.6%,切开复位克氏针内固定组71.4%;22例肘内翻行髁上截骨克氏针固定,优良率100%。结论:儿童肱骨髁上骨折闭合穿针及切开复位克氏针治疗疗效优于手法复位外固定。肱骨髁上截骨加克氏针固定是治疗儿童肘内翻简便有效的手术方法。

Objective：To evaluate the effect of different methods to treat child humeral supracondylar fracture,and to probe the methods to prevent and treat cubitus varus.Methods：To have follow-up on 122 children patients with humeral supracondylar fracture and 22 children patients with cubitus varus,and to assess the clinical curative effects.Results：Among 122 cases,the excellent rate was 50% in the group treated by manipulative reduction and 78.65% in the group treated by close pinning fixation and 71.4% in the group treated by open reduction and Kirschbner pinning fixation.22 cases of cubitus varus were treated by supracondylar osteotomy and Kirschbner pinning fixation,with the excellent rate of 100%.Conclusion：The methods of close pinning fixation and open reduction and Kirschbner pinning fixation are superior to manipulative reduction in the treatment of child humeral supracondylar fracture.Humeral supracondylar osteotomy and Kirschbner pinning fixation are simple and effective operative methods used in child cubitus varus.
