无创双水平气道正压通气在急诊呼吸衰竭中的临床应用

Clinical application of non-invasive positive pressure ventilation in emergency treatment of respiratory failure

目的:观察急诊无创气道正压通气(NIPPV)对各种原因引起呼吸衰竭的临床疗效。方法:对46例各种原因引起呼吸衰竭伴有不同程度的意识障碍的急诊患者使用双水平气道正压(BiPAP)无创呼吸机面罩辅助通气,动态观察NIPPV治疗前和治疗后2h、4h的临床指标、动脉血气分析和神志。结果:治疗后患者的呼吸频率(RR)、心率(HR)、动脉血pH值、PaO2、PaCO2、SaO2及意识状态均较治疗前有明显的改善(P<0.01)。结论:急诊无创双水平气道正压通气治疗各种原因引起呼吸衰竭疗效确切。

Objective：To evaluate the value of non-invasive positive pressure ventilation（NIPPV） via facial mask in the emergency treatment for the patients with respiratory failure.Methods：46 patients with respiratory failure were enrolled.Subjects received assisted ventilation via facial mask by bi-level NIPPV.The clinical indexes,arterial blood gas parameters and conscious state were dynamically observed before treatment and at 2h,4h after treatment.Results：After treatment,subjects had significant improvement in HR,RR,pH,PaO2,PaCO2,SaO2 and conscious state（P0.01）.Conclusion：Bi-level non-invasive positive ventilation is an effective therapy for the patients with respiratory failure in emergency.
