浸润性乳腺癌病灶超声征象与腋淋巴结转移及肿瘤MVD关系 被引量:6

摘要 目的:探讨乳腺癌超声征象与腋淋巴结转移及肿瘤微血管密度(MVD)相关性。方法:浸润性乳腺癌患者95例,术前1周内进行乳腺超声检查,随机选取30例计数肿瘤MVD。将病灶超声征象与术后病理腋淋巴结状况、肿瘤MVD进行相关性研究。结果:术后病理结果显示腋窝淋巴结转移34例,未转移61例。肿瘤MVD与乳腺癌病灶超声探测到血管数目之间有相关关系(P<0.05);病灶>2cm组MVD值高(P<0.05)。多因素逐步Logistic回归分析显示病灶超声最大径>2cm、前后径/左右径>1及有毛刺征是预测腋窝淋巴结转移的独立性因素,方程模型为P=1/[1+e-(-2.67+1.21x+1.14y+1.37z)]。结论:浸润性乳腺癌病灶血流丰富程度与肿瘤MVD呈正相关。病灶超声征象可用于预测腋淋巴结状况。 Objective:To study the correlation between tumor sonographic characteristics with axillary lymphnode metastasis and tumor microvessel density(MVD).Methods:The data from 95 patients with invasive breast cancer were obtained.The breast ultrasound examination was performed before 1 week of operation.30 cases were randomly selected for detecting MVD.Tumor sonographic features,postoperative pathogenic status of axillary lymphnodes and tumor MVD were performed for correlative analysis.Results:The postoperative pathology showed axillary lymphnode metastasis in 34 cases and no metastasis in 61 cases.The correlation existed between tumor MVD with vessel number detected by ultarsound in breast cancer focus(P0.05).MVD was higher in cancer focus2 cm(P0.05).Multivariate Logistic regression analysis showed that the maximal diameter of cancer focus2 cm,anteroposterior diameter/left-right diameter 1 and edge spicule sign were the independent factors predicting axillary lymphnode metastasis.Conclusion:The degree of blood flow abundance in invasive breast cancer has the positive correlation with tumor MVD.The sonographic features of cancer focus could predict the status of axillary lymphnodes.
