2型糖尿病胰岛素治疗方法的临床比较

Clinical comparison of insulin therapies for type 2 diabetes

目的探讨2型糖尿病短期胰岛素强化治疗的有效方法。方法 90例口服降糖药不能使血糖控制满意、需要胰岛素治疗的2型糖尿病患者,随机分为胰岛素泵组、甘精胰岛素组与低精蛋白锌胰岛素组,每组30例,观察各组患者治疗前后临床指标变化情况。结果胰岛素泵组和甘精胰岛素组空腹及餐后血糖、胰岛素剂量、不良事件、血糖达标时间及平均住院时间均低于低精蛋白锌胰岛素组(P<0.05);胰岛素泵组与甘精胰岛素组比较,差异无显著性(P>0.05)。结论甘精胰岛素组可作为口服药物难于使血糖控制满意的2型糖尿病患者强化治疗的首选方法之一。

【Objective】To investigate the effective methods in type 2 diabetes by short-term intensive insulin therapy.【Method】A total of 90 patients who had poorly controlled blood glucose by oral medicine were divided into 3 groups：insulin pump group（group A）,insulin glargine group（group B） and isophane insulin group（group C） with 30 patients in each group,and then the alterations of the clinical indicators before and after treatment were investigated in every group.【Result】The levels of fasting blood glucose,postprandial blood glucose,insulin dose,adverse events,compliance time of blood glucose,and the average length of hospital stay in group A and group B were both lower than those in group C（P〈0.05）.There was no difference between group A and group B（P〉0.05）.【Conclusion】Insulin glargine can be used as one of the intensive insulin therapies when the type 2 diabetic patients have poorly controlled blood glucose by oral medicine.
