立止血联合甘露醇控制超早期脑出血继续出血的疗效 被引量:1

Efficacy of reptilase combined with mannitol on continue bleeding after ultra-early intracerebral haemorrhage

摘要 目的探讨立止血、甘露醇联合应用控制超早期脑出血继续出血的临床疗效。方法将104例超早期脑出血病人随机分为对照组和治疗组,对照组按脑出血常规治疗,治疗组在常规治疗的基础上超早期立止血和推迟的甘露醇治疗,对两组病人发病24 h继续出血发生率和血肿大小及治疗21 d后临床疗效进行比较。结果脑出血病人早期继续出血发生率治疗组明显低于对照组,其中3 h内入院者两组比较差异有统计学意义,而3~6 h入院者两组比较差异无统计学意义;两组病人发病24 h血肿体积均增大,但对照组同治疗组相比增大更明显;治疗21 d后临床疗效治疗组明显好于对照组。结论立止血、甘露醇联合应用能有效控制超早期脑出血继续出血,改善预后。 Objective To observe the clinical effect of reptilase combined with mannitol in continue bleeding after ultra-early intracerebral haemorrhage.Methods One hudred and four patients with intracerebral haemorrhage were randomly divided into control group(group I) and treatment group(group Ⅱ).The patients in group Ⅰwere treated with conventional therapy scheme of intracerebral haemorrhage,the patients in group Ⅱ were treated with ultra-early reptilase and defermently mannitol besides the conventional therapy scheme.Then compared the rate of continue bleeding and size of haematoma after 24 hours and also the clinical effect after 21 days between the two groups.Results The rate of early continue bleeding in group Ⅱ was lower than that in groupⅠ,and the difference was significant between the two groups who were admitted into hospital within 3 hours.But there was no significant between the two groups who were admitted into hospital in 3-6 hours.The size of haematoma raised after 24 hours in the two groups,but the group I was more obviously than groupⅡ.And the clinical effect in groupⅡwas much better than that in group Ⅰ after 21 days.Conclusion Treatment of intracerebral haemorrhage by reptilase combined with mannitol can control ultra-early continue bleeding and ameliorate prognosis of patients.
