4种本虚证候蛋白尿患儿的中医辨证分型观察

The Traditional Chinese Medicine Syndrome Observation of Four Deficiency Types Children with Proteinuria

目的:探讨4种中医本虚证候在蛋白尿患儿中的分布及中医辨证施治在蛋白尿患儿中的治疗价值。方法:收集临床符合脾气亏虚型、气阴两虚型、肝肾阴虚型及脾肾阳虚型4种本虚中医证型的56例蛋白尿患儿,比较4种中医本虚证候在蛋白尿患儿中的分布情况。所有病例再随机分为中西医结合组及西医组,观察两组蛋白尿患儿治疗后的中医证型转变情况。结果:在符合要求的56例病例中,气阴两虚型最多,共28例占50%;肝肾阴虚型19例,占33.9%;脾气亏虚型7例,占12.5%;脾肾阳虚型仅有2例,占3.6%。在疾病分布上,儿童蛋白尿则以肾病综合征为主,其次为过敏性紫癜肾炎,二者证型在儿童同样以气阴两虚型及肝肾阴虚型居多。中西医结合组治疗后有9例患儿的虚损证型消失,肝肾阴虚型病例较前稍增,但气虚型及气阴两虚型有明显减少,与治疗前相比存在统计学差异(P<0.05)。相对于治疗前,西医组肝肾阴虚型增多,脾气亏虚型及气阴两虚型稍减少,治疗前后无统计学差异。结论:儿童蛋白尿患儿以气阴两虚型及肝肾阴虚型居多,脾肾阳虚型少见;中药辨证施治则对蛋白尿患儿的虚损证候有较好的调理作用。

Objective：To investigate the distribution of four deficiency types and the curative value of Chinese medicine on syndrome differentiation in Children with proteinuria.Methods：56 proteinuria Children with Pi qi-deficiency type,qi-yin deficiency type,Gan-Shen yin-deficiency type and Pi-Shen yang-deficiency type respectively were collected.Compared the distribution of four deficiency types in Children with proteinuria.All cases were random divided to two groups,the group of integrated traditional and western（group1） and the western group（group2） and observed the changes in TCM syndrome of two groups after treated.Results：Among 56 cases,the numbers of qi-yin deficiency type,Gan-Shen yin-deficiency type,Pi qi-deficiency type and Pi-Shen yang-deficiency type were 28 cases,19 cases,7 cases and 2 cases,respectively.In all collected disease,the most kinds were nephrotic syndrome and Henoch-Schoenlein Pur-pura Nephritis,and the mainly types were qi-yin deficiency type and Gan-Shen yin-deficiency type also.Compared with the group2,the deficieny syndrome of 9 cases were disappeared and the cases of Gan-Shen yin-deficiency type slightly increased,while the numbers of qi-yin deficiency type and Pi qi-deficiency type were significantly decreased（P 〈0.05）.Compare with before of treatment,it showed increased in Gan-Shen yin-deficiency and lightly decreased in Pi qi-deficiency type and qi-yin deficiency type.Conclusion：The mainly TCM Syndromes of Children with proteinuria were qi-yin deficiency type and Gan-Shen yin-deficiency type,and the Pi-Shen yang-deficiency type were sel-dom.There is sure value of Chinese medicine on syndrome differentiation in treat deficieny syndrome of Children with proteinuria.
