LASIK与PRK治疗高度近视的对比 被引量:5

A comparison between laser in situ keratomilleusis and photorefractive keratectomy for corection of high myopia

摘要 目的:为比较激光原位角膜磨镶术(LASIK)与准分子激光角膜切削术(PRK)治疗高度近视的临床效果。方法:随机选择随访半年且资料完整的LASIK(143只眼)和PRK(129只眼)两组治疗高度近视患者,并按屈光度分为A、B、C3个组,对两种术式在不同度数、不同治疗时期的术后视力、屈光度、并发症等进行系统的对比分析和统计学处理。结果:L式和P式术后6个月的裸眼视力≥05者分别为958%和798%(P<001);裸眼视力≥10者为741%和511%(P<001)。实际矫正度在预计矫正度±100D之间:L式分别为A组755%、B组553%、C组484%;P式为A组703%、B组457%、C组367%。手术后最佳矫正视力:提高者L式和P式分别为550%、364%(P<005),下降者分别为15%、77%(P<001)。结论:LASIK在治疗高度近视尤其是大于900D以上的高度近视方面,其术后视力恢复速度、疗效、手术范围、预测性、稳定性和安全性均优于PRK。 To compare the clinical effects or laser in situ keratomilleusis (LASIK) and photorefractive keratectomy (PRK) for the correction of high myopia 181 patients with high myopia,whose follow-up information for half year were obtained completely,were divided into 2 random groups (group P and group L).LASIK was performed on 143 eyes,PRK was treated on 129 eyes According to the refraction,the patients were subdivided into 3 groups:Group A,Group B,and Group C Comparisive analysis were made about postoperative vision,refraction degree and complication for two operative methods in different refraction degree and different treatment period The postoperative vision of group L and group P for 6 months were concerned,patients with vision≥0 5 were 95 8% in group L and 79 8% in group P(P<0 01);those with naked vision≥1 0,74 1% in group L and 51 1% in group P individually (P<0 01) Actual correction degree,which was within expectant corrections degree±1 00D in group L were 75 5% (group A),55 3% (group B) and 48 4%(group C);in group P,70 3%(group A),45 7% (group B) and 36 7% (group C) separetely Postoperatively the best correction vision,patients elevated in vision were 55 0% in group L and 36 4% in group P (P<0 05) those lowered in vision were 1 5% in group L and 7 7% in group P respectively (P<0 01).So far as postoperative vision,recovery speed and effection,operative scope,prediction,stability and safety were concerned,LASIK had advantage over PRK in treatment of high myopia,particularly for those with refraction over -9 00D
