MCM4、p53在子宫颈上皮内瘤变和子宫颈癌中的表达及其临床意义 被引量:10

摘要 目的探讨MCM4和p53在子宫颈上皮内瘤变和子宫颈癌中的表达及其临床意义。方法采用免疫组化EliVision两步法检测48例子宫颈癌、15例CIN1、27例CIN2~3及15例正常子宫颈黏膜上皮组织中MCM4和p53蛋白的表达,并结合临床资料进行分析。结果 MCM4、p53在正常子宫颈黏膜上皮、CIN1级、CIN2~3级与子宫颈鳞癌中表达阳性率分别为6.67%、20.00%、55.56%、83.33%和0、6.67%、18.52%、41.67%,差异均有显著性(P<0.05)。MCM4的表达与子宫颈鳞癌的病理学分级和临床分期有关(P<0.05),与年龄分组和淋巴结转移无关(P>0.05)。p53的表达与子宫颈鳞癌的病理学分级、临床分期、年龄分组和淋巴结转移均无相关(P>0.05)。MCM4和p53表达无相关(P>0.05)。结论 MCM4可以作为细胞增殖标志,用于子宫颈癌及其癌前病变的鉴别和诊断,并可评估肿瘤预后,指导临床治疗。 Purpose To investigate the MCM4 and p53 expression and their significance in cervical intraepithelial neoplasia (CIN) and cervical carcinoma.Methods MCM4 and p53 expression was examined by immunohistochemistry (EliVision method) in 48 cases of cervical carcinoma,15 cases of CIN 1,27 cases of CIN 2~3 and 15 cases of normal cervical epithelial tissues.Results The positive expression rates of MCM4 and p53 in normal cervical epithelium,CIN 1,CIN 2~3 and cervical carcinoma were 6.67%,20.00%,55.56% and 83.33%,and 0,6.67%,18.52% and 41.67%,respectively.The positive expression rate of MCM4 was significantly different between CIN and cervical carcinoma (P0.05).MCM4 expression was closely correlated with pathological grade of cervical carcinoma and clinical stage (P0.05).However,there was no significant association between MCM4 expression and p53 in cervical carcinoma and CIN (P0.05).Conclusions MCM4 is useful for evaluating cell proliferative activity.Potential applications of it include the identification and diagnosis of malignancy and precancerous lesions,assessment of prognosis and assistance of clinical management.
