我院门诊不合理药物联用处方分析

Irrational drug combination in the out-patient department of a hospital

目的了解我院门诊处方中药物不合理联用情况。方法随机抽查2009年6～12月门诊处方24 658张,根据药品说明书及相关文献资料,筛选出药物不合理联用的处方,并进行分类、统计及分析。结果两种以上药物联用处方10 722张,存在药物不合理联用处方553张,占所查处方总数的2.24%,其中单处方联用药品数在五种以上处方130张,抗微生物药+抗病毒药处方250张,杀菌剂+活菌制剂+双八面体蒙脱石处方32张,小儿复方四维亚铁散+维D钙咀嚼片处方30张,螺内酯+血管紧张素转化酶抑制剂处方9张,复方硫酸亚铁叶酸片+维生素C处方60张,抗微生物药之间不合理联用处方42张。分别在联用药品数量、药理作用、使用剂量等方面存在问题。结论医师与药师在工作中要相互沟通,并有合理的行政措施干预下,不断提高临床合理用药水平。

Aim To investigate the status of irrational drug combination from the out-patient department prescriptions.Methods 24658 prescriptions dated from June to December,2009 were chosen from the out-patient department randomly.Irrational drug were analyzed with the dispensatory and pharmacologic information.Results Of 10 722 prescriptions of drug combination,553 were irrational,accounting for 2.24% of the total 130 prescriptions had more than five kinds of drugs,250 with combinations of antibiotic and antivirus,32 with antibiotic,bacterium and smectite,30 with complex chalybeate for baby and calcium supplement with vitamin D chewable tablets,9 with spironolactone and ACEI,60 with complex iron sulfate VM and Vitamin C,42 with irrational antibiotics combination were found out to have problems on kinds,numbers and influence of the drugs.Conclusion Rational use of drugs should be improved by doctors,pharmacists and the leaders of the hospital with learning more knowledge and communicating with each other.
