后腹腔镜治疗肾上腺嗜铬细胞瘤临床观察

Clinical observation of treatment for adrenal pheochromocytoma by retroperitoneoscopic excision

目的探讨后腹腔镜下肾上腺嗜铬细胞瘤切除的安全性和实用性。方法回顾分析我院2005年8月～2009年8月后腹腔镜下手术切除的20例肾上腺嗜铬细胞瘤患者的临床资料。结果 20例手术均成功,无相关并发症发生。手术时间65～140min(平均90min),出血量30～200ml(平均75ml),瘤体直径2.5～6.4cm(平均4.5cm)。病理均提示嗜铬细胞瘤。结论术前准备充分,术中细致操作,后腹腔镜下对肾上腺嗜铬细胞瘤切除是安全有效的,腹腔镜手术可成为治疗肾上腺嗜铬细胞瘤的首选方法。

Objective To explore the clinical safety and efficacy of treatment for adrenal pheochromocytoma by retroperitoneoscopic excision.Methods The clinical data of 20 patients with adrenal pheochromocytoma who were treated by retroperitoneoscopic excision from August 2005 to August 2009 were analysed retrospectively.Results All the operations were completed successfully without relevant complications.Mean operative time was 90min（range 65～140min）.Mean estimated blood loss was 75ml（range 30～200ml）.The mean tumor size were 4.5cm in diameter（range 2.5～6.4cm）.All the 20 cases were pathologically diagnosed as pheochromocytoma.Conclusion As long as preoperative preparations are adequate and intraoperative manipulations are careful,retroperitoneoscopic excision of adrenal pheochromocytoma is safe and effective,retroperitoneoscopic excision has become primary option for adrenal pheochromocytoma.
