两种不同手术方式治疗胃十二指肠溃疡急性穿孔的疗效观察

Observing the Effect of Two Methods in Treating Patients with Acute Perforation of Gastroduodenal Ulcers

目的:探讨两种不同的方法治疗胃十二指肠溃疡急性穿孔的临床效果。方法:将2007年1月至2010年1月间在我院住院治疗的胃十二指肠溃急性穿孔患者60例分为观察组和对照组,对照组给予单纯修补术配合正规药物治疗,观察组给予胃大部分切除术治疗,随访治疗效果。结果:观察组患者在手术时间、住院时间、术后并发症方面明显低于对照组,两组比较差异具有统计学意义(P<0.05)。观察组患者在溃疡复发率方面与对照组比较差异无统计学意义(P>0.05)。随访研究发现观察组Ⅰ级、Ⅱ级、Ⅲ级及Ⅳ级溃疡复发率与对照组比较差异无统计学意义(P>0.05)。结论:单纯穿孔修补术治疗胃十二指肠急性穿孔具有诸多优点,不失为临床治疗的首选方法。

Objective： To investigate the effect of two operations in treating cases with acute perforation of gastroduodenal ulcers.Method： The 60 cases with acute perforation of gastroduodenal ulcers in our hospital from January in 2007 to January in 2010 were divided into the observing group and the control group,the observing group received the treatment of simple kposthesis combined with pharmacotherapy and the control group received the treatment of stomach premodinantly ablation.Result： The time of operation,postoperative complications and length of stay in the observe group was superior to that in control group,and there was significant difference between them respectively（P〈0.05）.The recurrence rate of ulcer in observing group was the same to that in control group,and there was no significant difference between them（P〉0.05）.The recurrence rate of Ⅰlevel,Ⅱlevel,Ⅲlevel and Ⅳlevel in observing group was the same to that in control group,and there was no significant difference in them（P〉0.05）.Conclusion： There are many merits in simple kposthesis on treating cases with acute perforation of gastroduodenal ulcers,so it is an ideal way in treating cases with acute perforation of gastroduodenal ulcers.
