高血压对肾动脉阻力指数影响的观察(附100例分析) 被引量:3

摘要 目的分析原发性高血压患者临床指标与肾动脉阻力指数(resistive index,RI)的相关性,研究高血压对肾动脉阻力指数的影响。方法选择原发性高血压患者100例。用彩色多普勒超声仪测量肾门动脉、肾段动脉和叶间动脉的RI,彩超检查的当天测量收缩压(SBP)和舒张压(DBP),计算脉压(PP)、平均血压(MBP);检测血清尿素氮(BUN)、血清肌酐(Scr);分析肾动脉RI与临床指标的相关性。结果高血压患者各级肾动脉RI与高血压分级、SBP、DBP、PP、MBP、年龄均有相关性,以肾门、段动脉相关性为大。BUN、Scr与各级肾动脉RI无相关性。结论原发性高血压患者的肾动脉RI与DBP、PP、MBP、年龄相关,可以间接反映高血压对肾动脉的影响及全身动脉硬化的状况,但需要考虑DBP的因素进行相应的评估。 Objective To investigate the changes of renal arterial resistive index(RI)in patients with primary hypertension,to analyze the correlation between renal arterial RI and clinical characteristics and study the hypertension how to affect the renal blood flow with normal renal function.Methods One hundred cases of primary hypertensive patients were selected.Renal arterial RI were detected in all cases by Color Doppler Flow Imaging.Systolic blood pressure(SBP)and diastolic blood pressure(DBP)were measured on the same day of examination.Pulse pressure(PP),mean pressure(MBP)was calculated.Blood urea nitrogen(BUN),serum creatinine(Scr)were measured in all patients.Results Renal arterial RI of various renal arteries in hypertension patients showed that renal arterial RI positively correlated with PP age and negative correlated with DBP,MBP.No correlated with SBP,BUM and Scr.Conclusion Primary hypertension patient's renal arterial RI correlated with DBP,PP,MBP,AGE,Renal arterial RI can reflect the state of arteriosclerosis in the body,but need to consider the affection induced by DBP.
