呼气末二氧化碳分压监测在机械通气中的应用 被引量:7

End-tidal carbon dioxide partial pressure monitoring in the application of mechanical ventilation

摘要 目的探讨呼气末二氧化碳分压(PETCO2)监测在机械通气中的应用价值。方法选择气管插管、机械辅助通气患者67例,在0、1、3、12、24h抽动脉血进行血气分析并记录即刻PETCO2。结果 67例患者在机械辅助通气治疗后0、1、3、12、24h,PETCO2与PaCO2分别呈正相关(r值0.794~0.916,P<0.01),随着病情的改善,PaCO2与PETCO2之差,即ADco2有所减少。结论 PETCO2监测是属于无创性监测方法,可连续监测,不仅能监测通气,还可以监测肺部循环功能,重复性好,在危重患者机械通气中有良好的临床应用价值。 Objective To observe the monitoring of end-tidal carbon dioxide partial pressure(PETCO2)in the patients who had mechanical ventilation.Methods Sixty-seven patients were intubated and ventilated mechanically with a monitor of PETCO2 continuously.Artery blood has been taken for blood analysis at 0,1,3,12,24 h,intervals.Results The PETCO2 and PaCO2 were positively correlated in 67 patients with mechanical ventilation(r=0.794-0.916,P0.01).The ADco2(PaCO2-PETCO2)reduced as the patients condition had been improved.Conclusion PETCO2 monitoring is non-invasive monitoring method,which not only monitors ventilation,but also monitors the circulatory function and pulmonary blood flow.It is reproducible and continuous monitoring.It has more valuable clinical practice for the critical ill patients.
