中心型肺癌肺静脉浸润的CT评价

CT evaluation on the extension of ceatral type Lung cancer to the pulmonary veins

ＣＴ评价中心型肺癌肺静脉侵润的价值。回顾性分析经手术的１５２例中心型肺癌术前ＣＴ扫描图像。２７例显示肺静脉侵润，并累及心包膜内，没有左心房内充盈缺损。１５例显示左右上肺静脉侵润，所有肿瘤都累及心包膜下。１２例显示左右下肺静脉侵润，７例有心包膜受累（５８％）。两者间统计学上有显著差异（Ｐ＜００５）。ＣＴ可应用于评价中心型肺癌肿瘤侵润范围。上肺静脉消失，高度提示肿瘤通过肺静脉累及心包内。相反，左右下肺静脉消失。

Objiective To evaluate the significance of extension of the central type lung cancer to the pulmonary veins with CT computerized tomography.Materials and Methdds:Preoperative CT scans of 152 patients with central type lung cancer which had been confirmed after operation were reviewed.Results:CT scans of 27 patients showed invasion of the puilmonary veins up to its entrance into the left or right superior pulmonary veins,and all tumors extended below the pericardium.Intrapericardial extension occurred in 9(58%) of 12 patients showing invasion of either the left or right superior pulmonary veins.The difference was statistically significant(P<0.05).Conclusion:In assessing extension of the central type lung cancer with CT,invasion of the superior pulmonary veins strongly suggest the intrapericardial extension a suggestive while invasion of the inferior pulmonary veins is believed to be a less reliable finding for that.
