脑多发性结核瘤的低场MRI诊断价值

Low-field MRI Diagnosis of Multiple Cerebral Tuberculomas

目的:探讨脑结核瘤的MRI特征及诊断价值。方法:通过分析21例经临床诊治及随访证实的脑结核瘤MRI表现,总结其影像学特征。结果:21例脑结核瘤患者其病灶均为多发性,未成熟结核结节T1WI呈等或低信号,T2WI呈高信号,注射对比剂后呈结节状强化。成熟结核瘤在T2WI呈典型的"靶征",增强后呈环形强化。不典型结核结节在T1WI和T2WI均未见明显异常信号,或部分病灶仅有长T1长T2水肿信号,增强后可呈点状、结节状或环形强化。结论:脑多发性结核瘤的MRI有特征性表现,具有诊断价值。

Objective： To determine the MRI imaging characteristics of multiple cerebral tuberculomas for its diagnosis in clinical practice.Methods： Documents of 21 patients with multiple cerebral tuberculomas who were identified by clinical diagnosis and follow-up were reviewed to determine their MRI imaging characteristics.Results： We found 21 patients whose multiple cerebral tuberculomas were multiple nodules.The MRI showed as following： Immature tuberculomas showed isointense or hypointensity signal on T1-weighted imaging and hyperintensity signal on T2-weighted imaging respectively,and nodular enhancement after contrast injection.Mature tuberculomas showed typical＂target sign＂on T2-weighted imaging,and annular enhancement.There was no abnormal signal on T1-weighted imaging or T2-weighted imaging in atypical tuberculomas,but particular lesions nodules on long T1 and T2 could have peripheral edema,and point-like,nodular or ring enhancement after contrast injection.Conclusions： There is characteristic performance of low-field MRI for diagnosis of multiple cerebral tuberculomas.
