SPECT盆腔血池定量显像对盆腔淤血综合征的诊断价值

Diagnostic Value of Pelvic Blood Image in Pelvic Congestion Syndrome

目的探讨盆腔血池显像（ＰＢＩ）对盆腔静脉淤血综合征（ＰＶＣＳ）的诊断价值，建立显像诊断标准。方法健康妇女３０例，年龄２７～４８岁，观察组１３０例，年龄２８～４９岁。于月经干净后１周左右，静脉注射９９ｍＴｃＰＹＰ７４０ＭＢｑ，３０，６０，９０，１２０分钟，应用ＳｉｍｅｎｓＳＰＥＣＴ，配以低能高分辨准直器，矩阵６４×６４，采集盆腔前位，后位各２分钟记数，勾画盆腔静脉丛感兴趣区和髂血管区感兴趣区，求出各自记数／象素（ｃｏｕｎｔ／ｐｉｘｅｌ）进行比较，求前、后平均数，以ｘ±ｓ表示，大于两个ｓ为异常。结果以腹腔镜为金标准，发现ＰＶＣＳ组各时间点比值均大于对照组（Ｐ＜００５），轻度淤血各时间点比值分别为：０５９＜０７１，０７２＜０８５，０７９＜０９７，０６７＜０８８，其灵敏度，特异性，准确性分别为９０５％，９２６％，９３２％，中度淤血各值为：０７２＜０８３，０８６＜０９８，０９８＜１１５，０８９＜１０９，９３８％，９４２％，９４８％，重度淤血：０８４＜，０９９＜，１１６＜，１１０＜，９７８％，９２４％，９６３％。结论ＰＢＩ对于ＰＶＣＳ而言，具有安全，准确，客?

Objection To demonstrate the value of SPECT pelvic blood image(PBI) in diagnostic pelvic venous congestion syndrome (PVCS).Methods Health women 30 cases,25～ 47 years old as contrast group.Observe group was 130 cases women,27～48 years old who were suspected of PVCS.Aforesaid women were injected 99m Tc PYP740 MBq after monthly 1 week,following 30 60 90 120 minutes those pelvic ANT and POS T were collected 2 minutes separately by Simens SPECT with low energy high separate collimator,matrix 64× 64.then made ROI within pelvic venous region and bigvenous region to gain the between ratio of counts/pixel.All data expressed as ±s .Celioscope conceived of as gold standard compare with PBI to demonstrate its diagnostic value.Results The ratio of count /pixel was higher in PVCS group than in constrast group ( P <0 05).PBI exam were quite sensitive,its sensitivity,speciality and accuracy was 94 2%,93 4% and 96 4%.PBI has high relativity determinating degree of PVCS with celioscope.In PVCS 1degree,2degree,and 3degree,ratio of count/pixel was separately 0 59<0 71,0 72<0 83,0 84<at 30 minute,0 72<0.85,0 86<0 98,0 99<at 60 minute,0 79<0 97,0 98<1 15,1 16<at 90 minute,0 67<0 88,0 89<1 09,1 10<at 120 minute.Those sensitivity was 90 5%,93 8% and 97 8%.Speciality was 92 4%,94 2% and 92 4%.Accuraty was 93 2%,94 8% and 96 3%.Conclusion PBI is a safest,accurate,impersonal,untraumatic and repeatable methods in diagnosis PVCS.
