小脑幕区脑膜瘤的临床特征和显微手术治疗振刚陈(附29例报告)

Clinical features of tentorial meningiomas and microsurgery for them(a report of 29 cases)

目的探讨小脑幕区脑膜瘤的临床特征及治疗策略。方法回顾性分析2004年10月至2009年10月收治的29例小脑幕区脑膜瘤患者的临床资料。结果 29例小脑幕区脑膜瘤中,SimpsonⅠ级切除19例,SimpsonⅡ级切除6例,SimpsonⅢ级切除3例,死亡1例。术后临床症状消失或缓解27例,动眼神经功能障碍1例。结论小脑幕区脑膜瘤临床表现以颅内压增高为主,合适的手术入路及术中对静脉窦的恰当处理对改善患者的预后有重要帮助。

Objective To investigate the clinical features and surgical treatment of tentorial meningiomas.Method The clinicaldata of 29 patients with tentorial meningiomas,who underwent microsurgery for the tumors in our hospital from October,2004 toOctober,2009,were analyzed retrospectively.Results Of 29 patients with tentorial meningiomas,19 received Simpson GradeⅠresectionof the tumors,6 GradeⅡ,3 Grade Ⅲ and 1 died.Of 28 patients who survived after the surgery,27 were improved in the clinicalsymptoms and 1 had dysfunction of oculomotor nerve.Conclusions The main clinical manifestation in the patients with tentorialmeningiomas is produced by intracranial hypertension.Selecting the proper operative approach and the proper treatment to the venoussinuses are high important to the improvement of the prognosis in the patients with tentorial meningiomas.
