外伤性膀胱破裂62例报告

Reports of traumatic bladder rupture on 62 cases

目的探讨外伤性膀胱破裂的诊断和治疗方法。方法回顾性分析62例外伤性膀胱破裂患者临床资料,对诊断、治疗方法及治疗效果进行总结和分析。结果手术治疗48例,治愈43例,治愈率89.6%。4例死于严重合并伤,1例死于感染性休克。保守治疗5例,均获痊愈。9例未及手术死亡。结论膀胱造影,特别是CT膀胱造影是膀胱破裂可靠的诊断方法。早期诊断、及时手术探查修补裂口、充分引流、合理抗生素应用是提高治愈率、减少并发症的关键。

Objective To investigate the methods of diagnosis and treatment of traumatic bladder rupture. Methods The clinical data of 62 patients with traumatic bladder rupture were retrospectively analyzed. Results 48 cases witb traumatic bladder rupture were treated with surgical methods,43 cases were cured, recovery rate was 89.6%. 4 cases died of severely associated injury, 1 patient died of infectious shock after surgery. All five cases treated with non - surgical methods were cured. 9 patients were not subjected to treatment and died. Conclusion Cystography, particularly computed tomography cystography is the reliable diagnostic method. Early diagnosis, promptly repaired bladder crack, sufficient drainage, antibiotics application are keys for successful treatment of traumatic bladder rupture.
