腹腔镜下套扎法在输卵管间质部妊娠保守手术中的应用

Loop ligature under laparosopy in conservative surgery for interstitial tubal pregnancy

目的:探讨输卵管间质部妊娠腹腔镜手术治疗的临床效果及手术技巧。方法:回顾性分析腹腔镜下套扎法治疗输卵管间质部妊娠21例的临床资料。结果:21例均采用腹腔镜下峡部切断、间质部套扎切除病灶方法顺利完成手术,其中保留患侧输卵管19例,无1例中转开腹,手术操作简单,手术平均时间(55.2±18.6)min,术中平均出血(67.5±32.6)mL,最大限度保留了远端输卵管长度。结论:腹腔镜下套扎法保留远端输卵管行病灶切除术是安全、可行的,手术操作难度低便于掌握。

Objective：To discuss the effect of laparosopy surgery on interstitial tubal pregnancy and the surgery skill.Methods：Clinic data of 21 cases with interstitial tubal pregnancy with loop ligature under laparoscopy were analyzed and summerized.Results：All cases received inferior isthmus cutting-off and focus resection by interstitial loop ligature,including 19 cases with ill lateral oviduct preserved.No case had conversion to open surgery.The mean surgery time were（55.21？？8.6）min,and the mean bleeding volume during surgery were（67.5？？2.6）mL.All cases had distal oviduct preserved as long as possible.Conslusions：Resection by loop ligature under laparoscopy with distal oviduct preserved is safe and feasible.The operation is easy to perform.
