LARs人工韧带治疗肩锁关节急性完全性脱位的临床观察

摘要 目的:观察"人工LARs韧带"(简称LARs韧带)固定治疗肩锁关节急性TossyⅢ型(完全性)脱位的临床疗效。方法:选择第二军医大学长征医院2008年1月~2009年12月应用法国产LARs韧带治疗急性肩锁关节完全性脱位10例,所有病例均用LARs韧带固定、修复喙锁韧带。选择Karlsson肩关节功能评分。结果:经过术后6~12个月规律随访,术后2~3周所有病例肩锁关节功能基本恢复正常,优良率100%;术后X线拍片无肩锁关节再脱位;本组患者术后切口未出现明显疼痛、肿胀及渗液反应。结论:应用LARs韧带治疗肩锁关节急性完全性脱位可恢复解剖重建,恢复肩锁关节稳定性,有利于早期功能锻炼和肩关节功能恢复。 Objective: To observe the curative effect of the ligament advancement reinforcement system (LARs) artificial ligaments in treatment of the acute dislocation of Tossy Ⅲ type acromioelavieular joint. Methods: Ten eases with Tossy Ⅲ type acromioclavicular joint acute dislocation treated with LARs artificial ligaments (produced in France) in Changzheng Hospital, Second Military Medical University of Chinese PLA from January 2008 to December 2009 were selected. The broken coracoclavicular ligament was repaired directly by LARs artificial ligaments in all cases. The outcomes were evaluated according to Karlsson's criteria. Results: All cases were followed up for 6 to 12 months, and all dislocated acromiocla vicular joints returned to normal 2-3 weeks after operation. Excellent and good rate of shoulder function was 100% according to Karlsson standard. X-ray imaging showed all acromioclavicular joints had no redislocation. All cases had no obvious pain, swelling or exudation. Conclusions: The anatomical reconstruction and the stability of acromioclavicular joint dislocation can be obtained after operation with LARS artificial ligaments, h is also helpful in early exercise and recovery of shoulder joint function.
