摘要
目的探讨重症肌无力多次危象的诱因和预后。方法对65例重症肌无力危象患者(其中11例为多次危象患者)的临床资料进行回顾性分析。结果 11例多次危象患者共发生24次危象,且均有明确诱因,其中以感染(17例次,70.8%)与疲劳紧张(5例次,20.8%)最多见。多次危象组患者经大剂量甲泼尼龙冲击或联合大剂量免疫球蛋白治疗,均取得较好的疗效,预后与单次危象组比较差异无统计学意义。多次危象组患者并发抑郁焦虑状态的比例明显高于单次危象组(P<0.01)。结论重症肌无力多次危象的主要诱因为感染、疲劳紧张等。如避免上述危险因素,加强并发症的防治,并予以适当的心理治疗,重症肌无力多次危象可防可治。
Objective To study the predisposing causes and prognosis of multiple myasthenia gravis crisis （MGC） . Methods Clinical data of 65 patients with MGC （including 11 cases of multiple MGC） were analyzed retrospectively. Results Crisis of 11 patients with multiple MGC happened 24 times. And each time had its predisposing cause. Among which,infection （17 times,70. 8% ） ,fatigue and tension （5 times,20. 8% ） were the most. Patients of multiple MGC group all get good curative effects after large dose of Methyllprednisolone therapy or combination therapy of which and large dose of immune globulin. There was no significant difference of prognosis between multiple MGC group and single MGC group. The proportion of complicating depression and anxiety in multiple MGC group was significantly higher than single MGC group （P 0. 01 ） . Conclusions The main predisposing causes of multiple MGC are infection,fatigue and tension,et al. If avoid above risk factors,enhance the prevention and cure of complication and get right psychotherapy,multiple MGC can be prevented and cured.
