摘要
目的研究重症肌无力(MG)对执行功能的影响。方法采用数字符号转换测验(SDMT)、连线测验(TMT)、Stroop色词测验(SCWT)、言语流畅性测验(VFT)等执行功能认知测验,对36例全身型MG患者(全身型组)、33例眼肌型MG患者(眼肌型组)及36名健康体检者(正常对照组)进行执行功能检查。结果与正常对照组比较,全身型组SDMT评分明显降低(P<0.01),TMT-1计时、TMT-2计时和错误提醒次数的评分均明显增高(均P<0.01),SCWT(A、B、C)说错个数和SCWT(C)计时的评分均明显增高(均P<0.01),VFT动物及家中用品计时的评分明显增高,正确个数的评分明显降低(均P<0.01);眼肌型组VFT动物计时的评分明显增高,家中用品正确个数的评分明显降低(均P<0.01)。与眼肌型组比较,全身型组SCWT(B、C)说错个数的评分均明显增高(均P<0.01)。结论 MG患者存在执行功能障碍,提示MG可能存在中枢神经系统额叶的损害。
Objective To study the influence of myasthenia gravis(MG) on executive function. Methods The executive function of 36 patients with the general type of MG (general type group) ,33 patients with the Eye-Muscles type of MG (Eye-Muscles type group) and 36 normal controls (normal control group) were measured by the symbol digit modalities test (SDMT) ,trail making test (TMT) ,Stroop color words test (SCWT) and verbal fluency test (VFT) . Results Compared with normal control group,the score of SDMT in general type group was significantly decreased (P 0. 01 ) ; the scores of TMT-1 time,TMT-2 time and wrong times in general type group were significantly increased (all P 0. 01) ; the scores of SCWT (A,B,C) wrong times and SCWT (C) time in general type group were significantly increased (all P 0. 01) ; the scores of VFT (animal and home things) time in general type group were significantly increased,and the scores of VFT (animal and home things) right times in general type group were significantly decreased (all P 0. 01) ; the score of VFT (animal) time in Eye-Muscles type group was significantly increased,and the score of VFT (home things) right times in Eye-Muscles type group was significantly decreased (all P 0. 01) . Compared with Eye-Muscles type group,the scores of SCWT (B,C) wrong times in general type group were significantly increased (all P 0. 01) . Conclusion MG patients always have some disorder on executive function,which implyings that there may be impairments on the frontal lobe of central nervous system.
