基底动脉尖综合征的危险因素及其与预后的相关性

Risk factors and their correlation with prognosis of top of the basilar artery syndrome

目的研究基底动脉尖综合征(TOBS)的危险因素及其与预后的相关性。方法收集47例TOBS患者与132例大脑中动脉(MCA)脑梗死患者的临床资料,比较二者的脑卒中危险因素,并对TOBS患者进行改良Rankin量表(mRS)评分以评估预后,分析TOBS患者预后与危险因素的相关性。结果 TOBS组房颤发生率(36.2%)显著高于MCA梗死组(14.4%)(P<0.05)。TOBS组入院30d时病死率为10.6%;其病灶部位、病灶个数及并发症与预后显著相关(P<0.05～0.01)。结论房颤是TOBS发生的重要危险因素,而与TOBS预后无关。TOBS患者病灶部位、病灶个数及并发症均与预后密切相关。

Objective To research the risk factors and their correlation with prognosis of top of the basilar artery syndrome （TOBS） . Methods The clinical data of 47 TOBS patients and 132 patients with middle cerebral artery （MCA） infarction were collected. Their risk factors for stroke were compared,and the TOBS patients were scored by modified Rankin scale （mRS） to assess their prognosis. The correlation between prognosis and risk factors of TOBS were analyzed. Results The incidence of atrial fibrillation in TOBS group （36. 2% ） was significantly higher than that in MCA infarction group （14. 4% ） （P 0. 05） . At 30 d after admission,the case fatality rate of TOBS group was 10. 6% ,and the location and number of infarction focus and complications were all significantly correlated with prognosis （P 0. 05 - 0. 01） . Conclusions Atrial fibrillation is an important risk factor of TOBS, but not correlated with the ptognosis of TOBS. The location,number of infarction focus and complications of TOBS patients are all closely correlated with prognosis.
