脑出血患者急性期血肿扩大的危险因素

Risk factors of hematoma enlargement in patients with intracerebral hemorrhage at acute stage

目的探讨脑出血患者急性期血肿扩大的危险因素。方法对100例急性脑出血血肿扩大患者和100例无血肿扩大急性脑出血患者临床资料进行比较,并采用多元Logistic回归分析进行危险因素分析。结果两组患者的平均动脉压、病程、血肿形态、出血部位、出血量、呕吐、血浆纤维蛋白原水平差异有统计学意义(P<0.05～0.01);多元Logistic回归分析显示影响血肿扩大的因素是平均动脉压、病程、血肿形态、呕吐。结论平均动脉压升高、病程缩短、血肿形态不规则、频繁呕吐是脑出血患者早期发生血肿扩大的主要危险因素。

Objective To investigate the risk factors of hematoma enlargement in patients with intracerebral hemorrhage at acute stage. Methods The clinical data of 100 acute intracerebral hemorrhage patients with hematoma enlarged and another 100 acute intracerebral hemorrhage patients without hematoma enlarged were compared,and the risk factors were also analyzed by multivariate Logistic regression analysis. Results The mean blood pressure, interval to onset,hematoma shape,location,bleeding volume,vomiting,fibrinogen level were significantly different between the two groups （P 0. 05 - 0. 01） . The multivariate Logistic regression analysis showed that factors of influence on hematoma enlargement were mean arterial blood pressure,interval to onset,hematoma shape and vomiting. Conclusion High level of mean arterial blood pressure,short time of interval to onset,irregular shape of hematoma and frequent vomiting are main risk factors of hematoma enlargement in patients with intracerebral hemorrhage at acute stage.
