摘要
目的探讨喉鳞状细胞癌(laryngeal squamous cell carcinoma,LSCC)、喉鳞状上皮内瘤变(squamous intraepithelial neoplasia,SIN)中肿瘤转移抑制基因KAI1/CD82表达的临床意义。方法采用免疫组织化学染色EnVision法检测26例LSCC,34例喉SINⅠ~Ⅲ级和10例正常喉组织中KAI1/CD82蛋白的表达和分布,并结合患者的临床病理资料对其结果进行对比分析。结果 LSCC中KAI1/CD82阴性、弱、中、强表达依次为57.7%(15/26),26.9%(7/26),15.4%(4/26),0%(0/26),与喉SIN组和正常喉组织组相比,KAI1/CD82在LSCC的表达存在显著下调(P=0.0005)。LSCC中KAI1/CD82表达与TNM分期、病理分级、颈淋巴结转移均无关(P>0.05)。喉SIN中KAI1/CD82表达出现下调趋势,但与正常喉组织组相比差异无统计学意义(P>0.05)。结论 KAI1/CD82表达的改变是LSCC发生、发展中的早期事件,喉SIN向LSCC转化过程中KAI1/CD82可能起非常重要的作用,KAI1/CD82可作为预测肿瘤转移潜能的指标之一。
Objective To explore the clinical significance of the expression of KAI1/CD82 in the tissues of laryngeal squamous cell carcinoma (LSCC), laryngeal squamous intraepithelial neoplasia (SIN) Ⅰ to Ⅲ and normal laryngeal mucosa. Methods Envision immunohistochemistry was used to detect the expression of KAI1/CD82 gene in 26 specimens of LSCC, 34 specimens of laryngeal SIN Ⅰ to Ⅲ, and 10 specimens of normal laryngeal mucosa. Correlation of the expression of KAI1/CD82 gene to clinicopathologic data were statistically analyzed. Results The rates of negative, weak, moderate, and strong expression of KAI1/CD82 in LSCC were 57.7% (15/26), 26.9% (7/26), 15.4%(4/26), and 0%(0/26), respectively, significantly lower than those in normal laryngeal mucosa and laryngeal SIN (P=0.000 5). Expression of KAI1/CD82 in LSCC had no correlation with clinical stage, tumor histological grade and lymph node metastasis (P0.05). There was no correlation between the expression of KAI1/CD82 in laryngeal SINⅠ~Ⅲ and histological grade (P0.05) . Conclusion The change of KAI1/CD82 expression is an early event in tumorigenesis of LSCC. KAI1/CD82 may play a significant role from laryngeal SIN to LSCC. Its down-regulation may be considered as a potential marker for tumor metastasis.
