胸腔置管白介素-2联合顺铂治疗恶性胸腔积液的临床观察

A Clinical Observation of the Therapy for Malignant Hydrothorax by Intra-thoracic Injection Using IL-2 Combining with DDP

目的:探讨白介素-2(IL-2)联合顺铂(DDP)胸腔内注射治疗恶性胸腔积液的近期疗效和毒副作用。方法:62例恶性胸腔积液患者,随机分为两组。观察组31例,胸腔注射白介素-2及顺铂;对照组31例,胸腔内注射单药顺铂,均为每周1次,共1次～4次,观察两组患者近期疗效及毒副作用。结果:观察组总有效率为90.3%,高于对照组的68.1%,差异具有统计学意义(P<0.05)。毒副作用有胃肠道反应、骨髓抑制、胸痛及发热。观察组胸痛及发热稍高于对照组,但差异无统计学意义(P>0.05)。结论:胸腔置管白介素-2联合顺铂治疗恶性胸腔积液疗效好,毒副作用轻,值得临床推广使用。

Objective： To explore the short term effects and complication of IL-2 combining with DDP intra-thoracic injection for malignant hydrothorax. Methods： 62 cases of malignant hydrothorax patients were randomly allocated into 2 groups, in which 31 cases in observe group using IL-2 combining with DDP intra-thoracic injection, and 31 cases in control group using DDP alone intra- thoracic injection. The regime of every week for 1 - 4 weeks was used to observe the short term effects and complications. Results： The total response rate in observe group was higher than that in control group （90. 3% vs 68. 1% ） , which had significant difference （P 〈 0.05 ）. The complications included gastrointestinal tract reaction, bone marrow inhibition, chest pain and fever. The incidence rates of chest pain and fever in observe group was slightly higher than that in control group, but there was no significant difference （ P 〉 0. 05）. Conclusion： The IL-2 combing with DDP intra-thoracic injection for malignant hydrothorax has the features of good therapeutic effects and slight poisonous side effects, which is worth to be used extendly in clinic.
