肝细胞肝癌Cathepsin B、uPA和uPAR蛋白高表达的意义 被引量:2

摘要 目的 探讨肝细胞肝癌组织中组织蛋白酶B(cathepsin B,CB)、尿激酶型纤溶酶原激活物(urokinase-type plas-minogen activator,uPA)及其受体(wrodinase-type plasminogen activator receptor,uPAR)的蛋白表达量.在肿瘤侵袭转移中的作用及机制.方法 应用免疫组化法测定71例肝组织标本,包括对照组(NC组)12例、肝细胞肝癌组(HCC组)34例和癌旁组织组(PTT-组)25例的CB、uPA和uPAR蛋白表达量,分析蛋白表达量之间的相关性以及与临床病理因素的关系.结果 (1)HCC组CB、uPA和uPAR蛋白表达量均明显高于NC组和PTT组,差异有统计学意义(均P〈0.01),PTT组与NC组间差异无统计学意义(P〉0.05).(2)CB与uPA(r=0.793),CB与uPAR(r=0.766),uPA与uPAR(r=0.892)之间均存在正相关关系(均P〈0.01).(3)CB、uPA、uPAR在包膜不完整、有肝内或肝外转移者肝癌标本中的表达量明显高于包膜完整、无肝内或肝外转移者肝癌标本中的表达(均P〈0.05);与肿瘤的大小无关(P〉0.05);随肝癌分化程度降低,3项指标均有增高的趋势,但无统计学意义(P〉0.05).结论 肝癌组织CB、uPA和uPAR蛋白均呈现高表达,它们之间可能相互诱导,协同促使肝癌的侵袭和转移. Objective To determine the expression of cathepsin B (CB), urokinase-type plasminogen activator (uPA) and its receptor (uPAR) in hepatocellular carcinoma and its association with invasion and metastasis of the tumor. Methods The expression of CB, uPA and uPAR proteins was detected by immunohistochemistry in 34 specimens of hepatocellular carcinoma (HCC group) tissue, 25 specimens of paratumor tissue (PTT group) and 12 specimens of normal liver tissue (NC group). The association of above protein expression in HCC with clinicopathological features was analyzed. Results (1)The expression of CB, uPA and uP- AR was significantly higher in HCC than that in NC and PTT (all P 〈0.01), there were no significant difference between PTT and NC (all P 〉0.05). (2)There was a correlation between the expression of CB and uPA (r = 0.79), CB and uPAR (r = 0.766), uPA and uPAR (r = 0.892) (all P 〈0.01). (3)The expression of CB, uPA and uPAR in HCC with capsule invaded, intrahepatic or extrahepatic metas- tasis was significantly higher than those with integral capsule, without intrahepatic or extrahepatic metastasis (P 〈0.05). The ex- pression of CB, uPA and uPAR in HCC was not correlated with the tumor size and Edmondson grade of HCC (all P 〉0.05). Conclu- sion HCC highly expresses CB, uPA and uPAR proteins, which are also associated with the invasion and metastasis of HCC.
