床旁中路穿刺法颈内静脉置管体会

Experience of mid way internal jugular vein catheterization in bedside

目的:探讨床旁中路颈内静脉穿刺法中心静脉置管应用于心胸外科患者治疗中的体会。方法:300例心胸外科患者均采用右侧颈内静脉中路穿刺法静脉置管,观察穿刺成功率、不良反应发生率。结果:一次穿刺成功率285例(95%),12例(4%)反复穿刺后改其它穿刺径路成功,2例(0.67%)穿刺针进入颈总动脉,1例(0.3%)发生气胸,全部患者无空气栓塞、心律失常、心包填塞、感染等并发症发生。结论:右侧颈内静脉中路穿刺法置管,定位简单,穿刺深度浅,穿刺迅速,成功率高。

Objective：Experience of mid way internal jugular vein catheterization in bedside.Methods：300 cases mid way internal jugular vein catheterization in bedside,Clinical Study on successful rate and incidence of adverse reactions.Results：the achievement ratios of once puncture were 95.3%（286/300）,alteration to other pathway in 12 cases,The accuracy was 4%,puncture needle entered into cervical artery were 0.67%（2/300）,Penmothorax occurred in 1case.The accuracy was 0.3%.Conclusion：method of mid way internal jugular vein catheterization in right side was simple to positioning,rapidly to operation,the depth of puncturing was shallow and achievement ratio was high.
